Wrap Up
You must complete all of the Steps listed below by June 29th, 2007 in order to qualify for winning the Prizes.
Step 1.
Make sure all your Activities get entered in to your Get Fit Journal, whether manually or on-line.
Step 2.
Contact the same Clinic and take your final fun Keiki Fitness Assessment. Also, if you've been filling-out your Get Fit
Journal manually, then bring that to the Clinic with you.
The Clinic Will
Give you the final fitness assessment.
Fill in the bottom portion of the Application that is titled: MEDICAL INFORMATION
Send in the Application for you. Also, if you've been filling-out your Get Fit Journal manually, then they will send that in for you as well.
